Stroke is a leading cause of death and long term disability in the U.S. and around the world.  When it comes to treating stroke, time is brain. For every minute that stroke diagnosis and treatment is delayed, millions of more brain cells die and the likelihood of recovery to independent living dwindles. The most effective treatment for acute ischemic stroke is the clot busting medicine, tPA, which is only safe and effective in a narrow time window. Unfortunately, less than 5% of all stroke patients ever receive this life-saving drug, largely due to delays in stroke diagnosis and timely hospital arrival. This deficit in timely stroke treatment is even more profound for patients living in rural and underserved areas, including the surrounding regions of central and southwest Virginia, where proximity to a comprehensive stroke center is limited and emergency transport times are prolonged. 

The iTREAT team, including members of the UVA Stroke Neurology Division, Department of Emergency Medicine, Center for Telehealth, and the Thomas Jefferson Council for Emergency Medical Services (EMS), has developed a mobile telemedicine toolkit to reduce stroke onset-to-treatment times for patients in our region. Translating low cost, off-the-shelf mobile technology in an innovative way, we are equipping regional EMS providers to perform audiovisual teleconsultation with stroke specialists during ambulance transport. This novel method will allow us to more rapidly assess and diagnose stroke patients prior to hospital arrival, while taking advantage of otherwise idle transport times often exceeding 30 minutes in our rural area. To facilitate mobile video teleconferencing, we have partnered with Verizon Wireless and Cisco Systems to utilize high speed (4G) wireless cellular connectivity and high quality audiovisual streaming during patient transport. Standard equipment includes a tablet-based device (i.e. Apple iPad), secure videoconferencing platform, high speed modem, and magnetic external mount antenna – total bargain cost of about $5000 per ambulance agency.  

This novel approach could substantially reduce stroke onset-to-treatment times largely impacting the lives of individual stroke patients and their families. The prospects of our iTREAT mobile telemedicine toolkit offer a new infrastructure for the way the medical community approaches timely stroke care and other emergency medical conditions going forward.  No longer will we wait for the patient to get to the hospital, but now we can bring the hospital to the patient!
